تأثیر 10 هفته تمرین مقاومتی کل بدن با دریافت مکمل عصارة بابونه بر سطح سرمی لیپوکالین-2 و نشانگرهای گلایسمیک زنان دیابتی نوع دو

چکیده

زمینه و هدف: دیابت نوع دو یکی از شایع‌ترین اختلالات سوخت‌وسازی در سراسر جهان است. با توجه به عوارض و مشکلات ناشی از دیابت، استفاده از دارو‌های گیاهی یک راه درمانی و کنترل‌کننده می‌تواند باشد. همچنین فعالیت ورزشی، جزء درمانی مهم دیابت نوع دو است. با توجه به اطلاعات محدود در این موارد، هدف این پژوهش، بررسی تأثیر 10 هفته تمرین مقاومتی کل بدن با دریافت مکمل عصارة بابونه بر سطح سرمی لیپوکالین 2 و نشانگرهای گلایسمیک زنان دیابتی نوع دو است.
مواد و روش‌ها: بدین منظور در یک کارآزمایی بالینی دوسویه کور، 44 زن دیابتی غیرفعال 40 تا 60 ساله (میانگین قد 45/159 سانتی‌متر، وزن 83/76 کیلوگرم، سن 25/51 سال، BMI 24/30 کیلوگرم بر متر مربع) به‌صورت داوطلبانه انتخاب و با آرایش تصادفی به چهار گروه 11 نفری شامل گروه تمرین+مکمل (T+Cham)، گروه تمرین+دارونما (T+P)، گروه بابونه (Cham) و گروه دارونما (P) جایگزین شدند. مداخله شامل 10 هفته تمرین TRX و سه جلسه در هفته (با شدت فشار 13 تا 17 بورگ) بود. گروه‌های مکمل و دارونما روزانه سه عدد کپسول 500 میلی‌گرم حاوی عصارة بابونه و پودر نشاستة ذرت دریافت کردند. پیش و پس از مداخله سطوح لیپوکالین 2، قند خون ناشتا، انسولین و مقاومت انسولینی اندازه‌گیری شد. داده‌ها با استفاده از آزمون‌های تحلیل واریانس، تعقیبی توکی و t همبسته در سطح معنا‌داری 05/0  P£ تحلیل شد.
یافته‌ها: میزان سطوح گلوکز خون، انسولین و نشانگر مقاومت انسولینی در هر سه گروه مورد بررسی نسبت به گروه دارونما کاهش معنا‌داری نشان داد (001/0= P). اما لیپوکالین 2، فقط در گروه‌های تمرین+مکمل و تمرین+دارونما نسبت به گروه دارونما کاهش معنا‌داری داشت (001/0= P). همچنین گروه تمرین+مکمل در مورد متغیرهای گلوکز خون، انسولین و نشانگر مقاومت انسولینی نسبت به گروه‌های تمرین+دارونما و گروه بابونه تفاوت معنا‌داری داشت (005/0 P £). افزون بر این، در مورد لیپوکالین-2، گروه تمرین+مکمل نسبت به گروه‌ بابونه تفاوت معنا‌داری داشت (005/0P £)، اما نسبت به گروه تمرین+دارونما تفاوت معنا‌داری وجود نداشت.
 نتیجه‏گیری: با توجه به یافته‌ها می‌توان نتیجه گرفت که انجام تمرین مقاومتی کل بدن با دریافت مکمل عصارة بابونه می‌تواند موجب کاهش سطوح قند خون ناشتا، انسولین، مقاومت انسولینی و لیپوکالین دودر زنان مبتلا به دیابت نوع دو شود و در جهت افزایش سلامت زنان مفید باشد. همچنین این مداخله می‌تواند به‌عنوان راهکار مکمل و غیردارویی در بهبود وضعیت بیماران مبتلا به دیابت نوع دو استفاده شود.

چکیده [English]

Background and Purpose: Type 2 diabetes is one of the most common metabolic disorders worldwide. Considering the complications and problems caused by diabetes, the use of herbal medicines can be a therapeutic and controlling method. Moreover, exercise is an important therapeutic component of type 2 diabetes. Given the limited information in these cases, the aim of the present study was to investigate the effect of 10 weeks of total body resistance training with chamomile extract supplementation on serum lipocalin-2 levels and glycemic indices in type 2 diabetic women.
Materials and Methods: For this purpose, in a double-blind clinical trial, 44 inactive diabetic women aged 40 to 60 years (mean, height, 159.45 cm; weight, 76.83 kg; age, 51.25 years; BMI 30.24 kg/m2) were voluntarily selected and randomly divided into four groups (n=11 per group), including the exercise + supplement group, exercise + placebo group, chamomile group, and placebo group. The exercise protocol consisted of 10 weeks of TRX training, 3 sessions per week (at an intensity corresponding to 13-17 Borg scale). The supplement and placebo groups received three 500 mg capsules daily containing chamomile extract and placebo (corn starch powder). Lipocalin 2, fasting blood sugar, insulin, and insulin resistance levels were measured before and after the intervention. Data were analyzed using ANOVA, Tukey's post hoc and paired t-tests.
Results: Blood glucose, insulin and insulin resistance index levels in all three study groups showed a significant decrease compared to the placebo group (p = 0.001). However, lipocalin 2 was significantly reduced only in the exercise+supplement and exercise+placebo groups compared to the placebo group (p = 0.001). Furthermore, the exercise+supplement group had a significant difference in blood glucose, insulin and insulin resistance index variables compared to the exercise+placebo and chamomile groups (p = 0.005). In addition, in the case of lipocalin-2, the exercise+supplement group had a significant difference compared to the chamomile group (p = 0.005), though, there was no significant difference compared to the exercise+placebo group.
Conclusion: Based on the findings of present study, it could be concluded that performing whole body resistance training with chamomile extract supplementation can reduce fasting blood sugar, insulin, insulin resistance, and lipocalin 2 levels in women with type 2 diabetes and be beneficial in improving women's health.
