Abstract
Background and Purpose: Heart rate variability (HRV) an indicator of autonomic nervous system function is consistently impaired in sedentary smokers. Given the elevated cardiovascular risk in this population and the limited research on the effects of aerobic exercise, this study aimed to compare the impact of 12 weeks of high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) on frequency-domain HRV markers in young male smokers.

Materials and Methods: This study was conducted on 30 sedentary male smokers. Inclusion criteria included physical activity of less than 90 minutes per week based on the IPAQ questionnaire and smoking at least five cigarettes per day during the past two years. The subjects continued to smoke during the study. The subjects were randomly assigned into three equal groups (n=10 per group) of high-intensity interval training (HIIT), moderate-intensity continuous training (MICT), and a control group. The mean values (±standard deviation) of body mass index were 23.23 ± 0.95, 23.06 ± 1.62, and 23.06 ± 1.62 kg/m2, in three groups, respectively. The intervention was implemented over 12 weeks (36 sessions). The HIIT consisted of 8 repetitions of one-minute running at an intensity corresponding to 85-95% HRR followed by one-minute active rest at 45-55% HRR, for the first 4 weeks, 10 repetitions from the fifth to the eighth weeks, and 12 repetitions for the ninth to the twelfth weeks. The MICT protocol consisted of 30-40 minutes running on treadmill running at 55-65% HRR for. The training volumes of the two groups were matched based on the exercise duration and cardiovascular load. The HRV frequency-domain markers were measured before and after the intervention by using a Holter for 90 minutes. Data analyses were performed by using Shapiro-Wilk, paired t-test, and ANOVA.

Results: The paired t-test showed that in both the HIIT and MICT training group, the levels of HRV parameters including TP, HF, and VLF increased significantly after the intervention (P
